Lost Implant Records: What Your Restorative Dentist Needs
- Look for data in discharge summaries, contracts and receipts.
- Ask the previous clinic about the system.
- The name of a country or a photo does not give exact identification.
The implant passport is lost and you need to clarify the system. There are ways to recover the information: old documents, a request to the clinic. But identification from a single X-ray is not guaranteed.

Which old documents may contain the needed data
Discharge summaries, contracts, certificates of work done, stickers with component markings, e-mails.
What to ask the clinic of the previous treatment
- The name of the system and the manufacturer.
- The size and the date of placement.
- Whether there is a copy of the passport.
Write to them so that the reply is kept on record.
What cannot be determined exactly from a country name or a photo
The country of treatment does not indicate the system, and one X-ray does not guarantee exact identification. So the dentist will not promise to identify the system from a photo alone.
How the compatibility check is agreed
The dentist assesses what data is needed and, if necessary, takes additional X-rays. Compatibility is checked before parts are ordered.
Document request form
- Full name and date of treatment
- Request to provide the name of the system
- Size and date of placement
- Copy of the implant passport
- Contact for the reply
- Date of the request
Send it to the clinic where you were treated.
Questions and answers
Where can I look for information about the installed system?
In discharge summaries, contracts, certificates, letters, and at the clinic where the implants were placed.
Is the name of a country enough to determine the components?
No. A country does not indicate a specific system.
